Whenever you encounter a condition that you feel is unsafe, please report the condition to the Environmental Health, Safety & Risk Management office. You may contact the office via telephone at 245-3616 or email at ehs@txstate.edu.

The preferred method is to complete the following form and email the completed form to the EHS&RM office. The form will provide us the information we require to start an investigation into your concern.
